Think I caught a bug? When I place a refnote callout that has its leader style set to none, it appears overly large and does not respect the scale / work area set.
If I change the refnote to have a leader style, the problem goes away. I looked through the blocks>graphics>callouts and everything appears normal?

Indeed you have.
The RefNote blocks are created in millimeter units, and it would appear the one without a leader is not getting scaled up 25.4 times for an imperial drawing.
This will be corrected with the next update, although it might not be for a week or two, as we are in the middle of a massive overhaul of our metric scaling (with this being a casualty from one of the early changes).
